The Dorothy M. Hamilton and Fred Hamilton Foundation has a closed application process, meaning the Trustees identify selected non-profit organizations which foster the mission of the Foundation and invite them to submit funding requests.
The invited organizations must then submit the following documentation and information for our review, before a final determination is made:
(1) Copy of the IRS 501(c)3 tax exempt letter and proof of current exempt status;
(2) Copy of the Certificate of Incorporation;
(3) Copy of the most current financial statement and most recently filed Form 990.
(4) A statement (not to exceed 4 pages) which includes:
- Identifying information - including the organization's tax ID # and contact information
- Organizational history and mission
- Description of the project or activities to be funded and the target population to be served
- Amount of funds requested and a proposed budget
